Your Safety
1. For reasons of safety and liability, a child who is not enrolled in JKids Discovery may not be dropped off and left at Chabad.
2. For reasons of safety and liability, no child may leave JKids with anyone other than a parent, legal guardian or someone who has been authorized by phone call or text to Rabbi Chaim or Mrs. Sarale Rivkin .
3. We expect parents to read all JKids communications in a timely manner and if a response is requested, we expect parents to respond promptly.
2. PAYMENT:
Form of payment through plan must be submitted in a timely manner, unless otherwise arranged with JKids directors.
A cancellation refund will be determined by case to case basis.
3. Food at JKids
Please send your child with a drink and snack (Meat free) to Jkids weekly.
4. MEDICAL CARE
In case of emergency, I hereby give permission to the physician selected by the JKids director, to hospitalize, to secure proper treatment for and to order injection, anesthesia, or other procedure deemed necessary for my child by an M.D. as named on this form or if unavailable another M.D.. Every effort will be made to contact the parent / guardian and emergency contacts first. Should it be necessary for the well being of the camper to utilize outside medical or dental services all expenses involved will be paid for by the Parent. To the best of my knowledge, my child is in good health and I will notify the camp if he/she is exposed to any infectious diseases. I understand that my child may be dismissed during a camp day, due to illness, at the discretion of the camp, and I agree to abide by the Director’s decision.
5. IMAGES, ETC..:
Permission is hereby given to use in promoting JKids program and in other ventures directly relating to JKids (i) digital, photographic and video images or likenesses of camper; audio of camper; and (ii) statements, articles, names, music, art, photographs, audio recordings, films and videos created by camper or originating from Camp or from a Camp-related activity.
6. ADMISSION PROCESS:

1. Complete and submit the application.
2. After you have successfully submitted your application it will be subject to review. A team member will reach out to schedule a follow up call to discuss your application and confirm it's approval. Once approved an invoice will be sent to complete your child's enrollment.
*If you are a returning parent there is no need to set up the phone call!

**JKids Discovery Program is geared towards children considered Jewish by Torah & Halachik Jewish law, regardless of affiliation status.
Chabad of Kirkland has the rights to withdraw an application if not meeting above requirements.
